Executive Team
Sara Marriott, MS, LPC
Chief Executive Officer

Sara has over forty years experience working in the mental health field. For over twenty years, she has worked for PSA. For ten of those years she was the Clinical Director and Vice President of Operations, and for over ten years she has been the CEO. Sara has a broad background of experience that ranges from working with children and adolescents to the SMI population. She has sat on the Best Practices Committee of the State and has been involved in bringing new and innovative programming to PSA. Sara, along with her staff, helped create Art Awakenings, a new and creative way in working with those with psychiatric disabilities. The motto that she works with is one that has helped create People, Service, Action. Sara is licensed by the State of Arizona as a Professional Counselor.
Andrew Shapiro, MBA
Vice President
Andrew Shapiro is an experienced leader with broad experience in developing and managing programs for both children and adults in the mental health community. A respected strategist who provides business and policy development, Andrew has a wealth of managed care and Medicaid experience. Creative thinking, business acumen and exceptional financial skills have made Andrew a trusted counselor and advisor.
Most recently, Andrew served as executive director of Ohio Choices for six years. He has a solid record of accomplishment in health care operations management. Under his leadership Ohio Choices diversified funding streams and positioned the organization to take part in Hamilton County’s federal systems of care grant. Prior to his work in Ohio, Andrew was the Chief Operating Officer for PSA and is excited to return to the agency.
Cassie Leavitt, MA, LPC
Chief Operations Officer

Cassie has over twenty years of experience working in the social service sector. This includes work within the criminal justice system, with children and families and the last eleven years within Arizona’s Maricopa County behavioral health community. She has over nine years of managerial experience and has been the Chief Operating Officer for PSA Behavioral Health since 2008. Cassie has a BA in Psychology, a MA in Forensic Psychology and a CAGS in Counseling. She is a Licensed Professional Counselor (LPC) in the State of Arizona.
Belinda Rouzaud, LCSW
Director of Program Development & Clinical Initiatives

Belinda has worked in the Maricopa County mental health community for over 19 years. She received her Master’s degree in social work from ASU and is a licensed clinical social worker (LCSW). Belinda has extensive training in psychosocial rehabilitation, substance abuse treatment, Gestalt Therapy and EMDR. She brings a wealth of knowledge and expertise in many areas including: systems management, leadership, clinical oversight and supervision. Belinda has worked as a therapist with a variety of populations including working with the Seriously Mentally Ill, chemical dependency/substance abuse, families, children, teens and adults. She worked for 9 years in clinical leadership roles at the RBHA and prior to that she worked in a variety of settings that include inpatient, outpatient, crisis, and residential with at risk teens.
Charlotte Webb
Director of Recovery Support Services
Charlotte Webb is a long time proponent of recovery and wellness. Holding a firm belief in recovery being possible for all, Charlotte has spent over ten years working in the mental health field in many capacities including peer (recover) support specialist, peer mentor, and director of several innovative programs.
Karen Puthoff
President of ArtAwakenings

Karen has dedicated her career to eradicating stigma related to behavioral illness. She has over 25 years of extensive experience in the behavioral health field as a senior manager in marketing and public relations, and as a chief operating officer of a behavioral health foundation. A graduate of Purdue University and St. Mary-of-the-Woods College, Karen has worked with the inpatient, residential and outpatient behavioral health programs in Arizona and Indiana. She is currently the President of PSA Art Awakenings and feels blessed to be part of such a talented team that promotes empowerment and recovery through creativity.
Freddy Cortez
Director of Human Resources & Training

Freddy has worked in the Human Resources field for more than 14 years. He began his HR career working for the State of New Mexico for five years, and eventually moved to Phoenix. He graduated from Texas Tech University in Lubbock, Texas in 1991 with a degree in Management. He continued his education with a Masters Degree in Public Administration from Western International University. He is also Professional Human Resource (PHR), Certified, and maintains his membership in the Society for Human Resources (SHRM), both national and local chapters. Freddy started working for PSA Behavioral Health Agency in July of 2002. He was and continues to be the only Human Resource Director the agency has had. Freddy worked closely with CEO, Sara Marriott, to create and maintain the Human Resource Department for PSA Behavioral Health Agency. He has overseen various departments in the agency, including Operations, Property Management, and Training Departments, but has always been primarily responsible for the Human Resource Department and its staff.
Kevin Eccleston, CHC
Director of Compliance and Regulatory Affairs

Kevin has over 15 years of diverse behavioral health experience in the state of Arizona having previously worked for the Regional Behavioral Health Authority in Maricopa County and a Community Mental Health Center Partial Hospitalization Program. He has extensive experience in the provision of direct clinical care, quality of care auditing, risk management mitigation, implementation and design of corporate compliance programming, and compliance oversight for applicable governing requirements. Certification in Healthcare Compliance by the Health Care Compliance Association, Member Health Care Compliance Association
John Mireles
Director of Diversity

John Mireles is currently Director of Diversity/Ombudsman with PSA Behavioral Health Agency. John Mireles is a Certified Trainer in Cultural Competency with the State of Arizona Dept. of Behavioral Health Services. John Mireles is an Ordained Elder/Pastor, Church of God in Christ. John Mireles has held many administrative positions in government, higher education, and community based organizations: Assistant Director of Minority Affairs/Health Careers Opportunity Program, Marquette University School of Dentistry; Equal Opportunity Specialist, U.S. Dept. of Housing and Urban Development, Office of Fair Housing and Equal Opportunity; Executive Director, Spanish Centers of Southeastern Wisconsin; Director of Community Organizing, Federation for Civic Action; Deputy State Director, Arizona League of United Latin American Citizens; Executive Director, Walkers Point Youth and Family Center; Director, Harambee Ombudsman Project; and Director, Latin American Chamber of Commerce of Wisconsin.
Kim Quiros
Director of Billing/IT
Kim has worked in the behavioral health field for over 15 years. She has extensive Claims experience, as well as Information Technology (IT) supervision. Kim has implemented the first Electronic Medical Record for the agency and continues to be dedicated to her work. She will graduate with her Business Administration degree from Ottawa University this spring, and plans to continue her education.
Bonnie Kolakowski
Comptroller

Bonnie has seventeen years of experience in the accounting field. She has managed the accounting department for some major corporations and owned some small businesses. Bonnie has experience in financial reporting, tax preparation, new corporation setup, and SOX compliance. Bonnie saved previous corporations over $100,000 by analyzing and implementing new accounting procedures to create a more efficient work environment. She has a Master’s in Business Administration and a Master’s in Accounting as well as a Bachelor’s degree in Accounting and is currently pursuing her CPA license.
Mary Rose
Chief Financial Officer

Mary has over twenty years experience working in the accounting field. She is an Australian CPA with a Bachelors Degree in Accounting from Australia and a Masters of Business Administration (Accounting) from the United States. Mary’s experience is broad having served as Chief Financial Officer for entities in the healthcare, software and manufacturing industries. Her broad experience also includes managing international operations. Mary sits on many non profit boards in varying capacities, and understands the unique challenges of nonprofit operations while being able to apply experiences gained in the for profit world.

Gayna McCrary Rowe
Board President

Gayna Rowe joined the PSA board in 2007. Gayna had worked twelve years with a Public Service Company of Colorado, a gas and electric utility, in their Public Relations Division. While there she ran the company’s corporate speaker’s bureau, did community relations, corporate contributions and finally functioned as speechwriter for the President and CEO of the company. She has also worked with the Los Angeles Metropolitan Transportation Authority as a Public Affairs Supervisor. She later went on to write speeches for the President and CEO of the MTA. Her last position was with a Fortune 100 company, Bell Atlantic, now known as Verizon. In that position she was a Public Affairs Manager who was responsible for a statewide initiative, World School, corporate contributions and advertising, for the state of West Virginia. Additionally, she was a corporate speechwriter for the President and CEO of the company. Gayna graduated with a B.S. in Journalism from Colorado State University.
Susan Dess
Board Member

Susan Dess joined the PSA Board in 2010. She is an independent health care consultant specializing in Medicaid programs and services. Prior to working as a consultant she served as the Senior Vice President of Health and Wellness at ValueOptions, Inc. Ms. Dess has over 30 years experience in the area of health care delivery, operations, utilization management, care and case management, and disease management in the domains of Physical and Behavioral Health. Trained as a Registered Nurse, she received her B.S.N. and M.S. from the University of Maryland. Throughout her career, Ms. Dess has held a variety of clinical and administrative positions in provider, commercial, and managed care environments. She is a member of the Pi Chapter of Sigma Theta Tau National International Honor Society for Nursing and received the St. Agnes HealthCare SuperNova award for Clinical improvement in Emergency Services, and the Department of Defense, Office of the Lead Agent, General’s Award for Customer Service to TRICARE Northeast.
Ron Harness
Board Member

Ron is a retired Honeywell Electrical Engineer and a Computer Science Major with multi-year experience in the computer and avionics research and development. Ron came to Arizona in 1965, transferring from General Electric. Ron joined the PSA board of Directors as a parent of a mentally ill adult and is able to give much valuable insight as to ways the agency may provide support for its clients and their families.
Mimi Bohlman
Board Member

Mimi joined the PSA board in 1991. Born in Moscow, Idaho, and growing up in Spokane, Washington, Mimi attended the University of Idaho. She became a flight attendant for West Coast Airlines. Mimi has been active in the ASU Faculty Wives Club, president one year; Tempe Jr. Women's Club; served on the board of the Tempe Center for Habilitation for approximately eight years and served as President for 2 years. She has been a member of the Zonta Club of the East Valley, originally the Zonta Club of Tempe, since 1978 and have served as President 3 times. Mimi has been a member of the Tempe Sister Cities Corp since 1971, and took the original exchange students to Skopja, Yugoslavia, now Macedonia, in 1972. Mimi has been active in her church in Tempe since joining in 1965. She served on the Tempe Community Council Agency Review board for about 5 years, and organized a women's auxiliary for the Tempe Salvation Army.
Lawn Griffiths
Board Member

Lawn Griffiths joined the PSA board in 1987. Lawn is active at University Presbyterian Church, where he has served four three-year officer terms as an elder, deacon and trustee. He has a Bachelor’s Degree in journalism from Iowa State University (1968) and a master’s degree from the Medill School of Journalism at Northwestern University (1972). He served in the Peace Corps in Paraguay (1968-69) and the U.S. Army (1969-71). He was an editor and columnist at the Waterloo (Iowa) Courier, 1972-1984, and then moved to the Tempe Daily News as city editor and later managing editor. In 1987, he moved to the Mesa Tribune, now the East Valley Tribune and Scottsdale Tribune where he has been the Town Crier columnist and more than 15 years as a Spiritual Life Editor. He has spent 35 years with the daily newspapers. He has served on about 15 community boards. Lawn has earned 70 awards in writing and community service. In 1992 he received the annual Outstanding Citizen Leadership Award from Tempe Leadership and in 1995 won Tempe’s highest honor, the Don Carlos Humanitarian Award. In 2004, the Tempe Historical Society named him one of the 35 “Tempe Living Legends.”

Hilary Bass
Board Member

Hilary has worked in the tourism her entire adult life and all higher education was in relation to hospitality. While working in the capacity of the Independent Business Travel Manager, Hilary became acquainted with PSA and the work at Tucson’s Art Awakenings. Working as a hotel Sales Manager has given Hilary experience working and interacting with a variety of people, under numerous situations. Hilary joined the Board, in 2011 to assist PSA to bring awareness to Mental Health Challenges and Substance Abuse, so those who seek treatment can find success.
