Supportive Living

Philosophy

Supported Living (SL) and Supporting Living Assertive (SLA) services are based on the recovery philosophy and psychosocial rehabilitation principles that strive to empower individuals to take responsibility for his/her own well-being. Staff will work closely with individuals to develop and implement treatment goals that are unique to them. The focus will be on empowering the individual to transition into independent living and integrating into a community of their choice.

 

 

Description

SL and SLA services are provided to adults with a serious mental illness and/or co-occurring disorders.  Services are provided in home, at the outpatient offices or in community settings depending on the individual needs and treatment goals of the person served. SL and SLA services are focused on providing individuals with a positive treatment experience.  PSA facilitates this environment through participation in rehabilitation activities, clinical services and education on community based resources.  The goal is to assist and empower persons served to, build positive relationships manage their own behaviors and achieve optimal functioning in the community of their choice.       

Staff are available to individuals up to 24 hours a day, seven days a week, based upon the individual’s needs. 

 

PSA provides the following services:

 

Supportive Living (SL):

 

Population served: Maricopa County SMI Title XIX clients, both co-occurring and non-co-occurring assessed to require up to 15 hours of structure/support daily.  Crisis response is available based on individual needs.

Services Provided: Placement in a community based apartment for the duration of treatment (rent paid by client, based on a sliding scale), group and individual counseling, family support, assistance in the self-administration of medications and independent living skills to encourage rehabilitation and reintegration into the community. 

 

Supportive Living Assertive (SLA):

 

Population served: Maricopa County SMI Title XIX consumers, co-occurring and non co-occurring assessed to require up to 24 hours of structure/support daily.

Services Provided: Placement in a HUD subsidized home or apartment setting for the duration of treatment (rent paid by client, based on client income), group and individual counseling, family support, assistance in the self-administration of medications and independent living skills to encourage rehabilitation and reintegration into the community.
